所有吸煙產品都有害健康，包括電子煙。煙油一般由丙二醇、甘油、添味劑、尼古丁及其他添加劑混合而成，所產生的氣霧含有多種有害物質及致癌物，如尼古丁、甲醛、乙醛、煙草特有亞硝胺、多環芳香烴、丙烯醛、丙烯酼胺、重金屬、揮發性有機化合物等。
所以看上去，市面有些電子煙店，好像有點排斥「加熱煙」，也許是歷史的包袱，不想昨天的我打倒今天的我！
電子菸生產商有時會將其包裝成戒菸產品。但目前並無公認的臨床報告能說明電子菸的長期影響。

與傳統紙菸相比，電子煙與加熱菸都會用到加熱裝置，但兩者原理相當不同。以加熱菸來說，燃燒主體是與紙菸外觀、內在都十分相似的菸彈，透過加熱器低溫加熱取代傳統打火機來控制溫度，減少菸草在不完全燃燒下可能產生的焦油及危害物質。
所以身上比較不會有令人反感的味道。但有些劣質煙油雖然便宜，味道就會比較不優質，內容物也可能相對不安全，風險相對起來較大。
